This person is responsible to provide quality service in the intake and care for the clients of the Transition House, and the general upkeep of the Transition House.
DUTIES & RESPONSIBILITIES:
- The Support Worker will, with respect to care of clients:
- Maintain strict confidentiality of client information and any information gained as a result of the employment relationship.
- Provide crisis intervention.
- Coordinate efforts with co-workers, children's support worker, outreach worker and house manager (e.g. arrange transportation and accompaniment to appointments with the outreach worker).
- Maintain detailed records, logs and client files.
- Provide information, support, and advocacy in a non-judgmental manner, remembering that the clients needs come first.
- Assist residents in meal preparation and clean-up.
- Set clear boundaries with clients.
- Ensure the safety and security of facility and clients.
- Be a positive role-model for life skills training (e.g. nutrition, cooking, house cleaning, budgeting, grocery shopping, parenting etc.).
- Facilitate resident meetings (e.g. menu planning, chores, shopping list).
- The Support Worker will, with respect to other duties:
- Maintain cleaning and general upkeep of house and yard.
- Responsible for detailed/deep cleaning when required, ie: residents rooms at time of discharge and extraordinary cleaning from time to time to meet health and safety standards.
- Follow the daily operational procedures of the house regarding security, client files, log and residents.
- Answer all telephone calls on the crisis line. Maintain calmness, ensure safety of caller, a client centered approach, confidentiality and non- judgmental listening.
- Follow policy and procedures as outlined in the Transition House Policy Manual.
- Keep all written reports, client information, files etc in an up to date manner.
- Debrief with relieving staff at the end of each shift. Update the relief staff on any pertinent issues and information gathered on your shift.
- Treat all staff and management with respect and act as a part of a team with the goals of the society in view.
- Attend job related educational training.
- Although attendance at staff meetings is not mandatory, it is a requirement of workers, to keep themselves, informed of the on-goings or changes resulting thereof.
- Report any critical incidences or injuries immediately, to the house manager.
- Perform any other job related duty as requested by the Director of Housing and/or the Executive Director.
